Faculty & Staff Recognition Nomination Form

Please complete all fields. Any faculty, student, or staff in the College may submit a nomination. The nominee’s resume/CV and a letter of nomination is required (except for staff awards, which require only the letter of nomination). One letter of support may also be included (in addition to the nomination letter). Letters of nomination should directly address the eligibility and criteria for the award.

NOTE:  Nominations for faculty and staff awards are due by February 15.  Nominations for post-doc, graduate student, and diversity awards are due by March 15.
